Sounds like he’d likely need sedation for treatment. I don’t know of anything herbal that’d knock him out sufficiently to be safe if he’s that problematic.
There are some long acting ear meds that can be put in and will treat him for a week or two.
Also, chronic ear infections in dogs are almost always secondary to allergies. Have your vet look into that as well.
