        93 year old female with congestive heart failure has pacemaker incision site unhealed.  The pacemaker corner is not buried under the skin completely with a corner protruding out of the incision site.  The pacemaker cannot be replaced because the skin it too fragile.  The site is infected.  The doctor wants to put her on continuous antibiotic therapy.  He made no recommendations for pre/ probiotic therapy to accompany the antibiotics.  Could I spray a poultice on the open wound?  Would the granulation grow over the pacemaker in the area that protrudes?  Good idea?  Not a good idea?
